Job description

The Christchurch Older Persons Community Mental Health Team are looking for an experienced Community Mental Health Nurse to join this team.

As a Mental Health Nurse, you will provide a comprehensive, accessible and responsive mental health service to adults and their families within a defined locality. This is primarily a care‑coordination role, where you will be responsible for supporting patients on their journey of recovery. You will be a committed, dynamic, motivated professional with excellent communication, organisational skills and the ability to prioritise your work. You should have relevant and recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with patients, their carers and their family.

  • Provide a comprehensive, accessible and responsive mental health service to adults and families within a defined locality.
  • Support patients on their journey of recovery through care coordination.
  • Apply engagement and recovery philosophies in working with patients, carers and their families.
  • Collaborate with a well‑established supportive team made up of a Psychiatrist, Nurses, Psychologists, a Social Worker, Administration and a Support Time Recovery Worker.
  • Work within the Dorset HealthCare Mental Health Strategy under the direction of the team leader and locality service manager.
  • Contribute to effective multi‑disciplinary working and liaison with community, primary and other secondary & tertiary services/agencies to provide holistic care.

This role is for 22.5 hours per week and requires a valid driving licence and a car for assessment and monitoring patients in their homes and other community settings.
