Are you passionate about making a meaningful impact in mental health and substance use services? We’re looking for dedicated professionals to join our dynamic team as a Clinician for Community Mental Health.

What We're Looking For:

  • Qualifications: Master's Degree in Social Work, Counselling Psychology or other relevant social or health sciences discipline.
  • Experience: At least two (2) years' recent, related experience working with clients and families with, or affected by, mental illness and substance use disorders, or an equivalent combination of education, training, and experience will also be considered.

Detailed Overview

In accordance with Fraser Health's Vision, Mission, and Values, the Mental Health & Substance Use program mandates and clinical practice guidelines, with an emphasis on recovery and rehabilitation and reducing barriers to re-enter the system of care, supports individuals and families with and affected by acute/episodic mental illness and substance use disorders, such as those with complex, unpredictable, and/or multi-faceted therapeutic needs. The focus is on providing intensive individual and group therapy, liaison regarding complex situations, and care coordination support across the care continuum to ensure timely and relevant treatment, rapid re-engagement, and access to information where appropriate and feasible. The role involves contributing to program development and providing community mental health education. Services may be provided on an outreach basis when required to assess and engage clients within their environment and community.

As a clinician in a shared-team model, you will provide intake, screening, assessment services, treatment recommendations and plans, determine referral needs, provide follow-up support, conduct group education, liaise with community resources for specialized services, counsel clients, provide outreach as needed, and be assigned as the "most responsible clinician" to specific clients based on their primary needs. You will deliver client and family-centered care through a trauma-informed and evidence-based approach, establishing trusting relationships, promoting self-determination, and empowering clients and supports to engage within their communities and navigate the healthcare system.

Responsibilities include:

  1. Providing treatment and support for complex situations, including crisis intervention, collaborating with clients and supports to develop recovery plans, and supporting activities of daily living, discharge, and transition of care.
  2. Offering short-term interventions, follow-up, and evaluating treatment outcomes.
  3. Conducting individual and group therapy, and family support using various counseling and assessment methods, sharing workload within the team, managing cases, and facilitating crisis intervention and outreach.
  4. Assessing client needs through initial screening, determining care priority, eligibility, resource availability, and appropriate treatment, and collaborating with the interdisciplinary team for optimal care approach.
  5. Arranging hospital admissions when necessary, including involuntary admissions, and coordinating transportation and documentation.
  6. Facilitating client utilization of community services and rehabilitative programs, including housing support and referrals to other community resources.
  7. Coordinating services with police, hospitals, crisis lines, and suicide prevention programs, and identifying service gaps to improve community mental health care.
  8. Engaging families and involving lived experience activities to promote knowledge exchange, providing education on mental health and substance use, and relapse prevention strategies.
  9. Providing information and guidance about services, policies, and procedures to clients, families, and the public.
  10. Maintaining accurate client records and documentation in accordance with standards and regulations.
  11. Advocating for clients, ensuring basic needs are met, and providing follow-up care and referrals to other health professionals.
  12. Promoting quality improvement, developing clinical protocols, and participating in team planning and community committees.
  13. Maintaining data on service requests and referrals, and contributing to clinical research and program evaluation activities.
  14. Performing other related duties as assigned, in accordance with health and safety requirements and community engagement goals.

Qualifications:

Master's Degree in Social Work, Counselling Psychology, or relevant social/health sciences. Two (2) years' recent experience working with clients with mental health and substance use disorders, or an equivalent combination of education, training, and experience. Eligibility for professional registration and a valid BC driver's license with access to a personal vehicle are required.

Skills and Abilities:

  • Knowledge of psychiatric assessment, crisis intervention, de-escalation, therapy techniques, psychopharmacology, and community resources.
  • Effective communication skills, both verbal and written.
  • Ability to prioritize, organize, and work independently.
  • Knowledge of violence de-escalation, conflict resolution, and group facilitation.
  • Proficiency with related equipment, computer literacy, and understanding of relevant legislation such as the Mental Health Act and FOIPP.
