We are looking for a highly motivated and enthusiastic individual to work closely with our Matrons, Clinical Nurse Manager and Charge nurses to facilitate the timely, effective and appropriate referrals to the Single Point of Access (SPA).

• Manage the Echo and 24-hour tape diary.
• Submit monthly Advice & Guidance data to the Information Team.
• Ensure all patient encounters are promptly booked on Cerner.
• Coordinate referrals to community agencies, fostering strong partnerships with external organisations including Primary Care Trusts, GPs and Social Services to facilitate safe, timely discharges and prevent unnecessary hospital admissions.
• Support the smooth day-to-day operation of the service, optimising hospital bed usage and reducing patient length of stay.
• Maintain professional and statutory practice standards in line with the Community Care Act, Council policies and Acute Trust guidelines.
• Demonstrate excellent communication skills and form positive relationships with ward staff and service users on the ward.
• Communicate with barriers to understanding, utilising interpreting resources when needed.
• Maintain effective partnership/joint‑working arrangements with external providers and wider voluntary sector services.
• Triage referrals, distinguishing between urgent and less urgent referrals within the SDEC pathways, ensuring that every referral is directed to the appropriate pathway.
• Report operational concerns regarding the SPA system to the SPA Senior Administrator or Manager.
• Provide complex information to patients/carers and request assistance where there are barriers to language or understanding, including obtaining informed consent in sensitive circumstances and demonstrating empathy with patients, carers and families.
• Support safeguarding and promote the welfare of children, young people and adults at risk.
• Undergo enhanced criminal records checks and comply with ongoing updates throughout employment.

• Work shifts covering service hours from 08:00 to 22:30, seven days a week to ensure continuity of care.
• Depending on demand you may be required to work Monday to Friday either 08:00‑16:00 or 10:00‑18:00.
• Be flexible to go where the need is greatest within the entire SDEC organisation.

• Uphold confidentiality of information consistent with legislation and Trust policies.
• Demonstrate skills and resourcefulness in communication when barriers to understanding are present.
