We are seeking a highly motivated and experienced Frailty Practitioner to join our expanding Frailty Service in the North Sector.
This key role within a multidisciplinary team supports the delivery of high-quality, person-centred care to older adults living with frailty. The Frailty Service in the North Sector is a dynamic model designed to meet the complex needs of this population. Based at Glasgow Royal Infirmary, the service utilizes the HIS Frailty Assessment Tool for early identification, provides rapid access to Comprehensive Geriatric Assessment (CGA), and involves coordinated multidisciplinary input. The team has demonstrated measurable improvements in patient outcomes, reduced hospital stays, and better discharge planning. They work closely with health and social care partners to deliver person-centred care and are committed to ongoing improvement, education, and innovation in frailty management.
As part of the GGC-wide network of Advanced Frailty Practitioner posts, this position will be central to developing and delivering this innovative service, supporting frail individuals to maximize their independence and wellbeing within their communities and prevent unnecessary hospital admissions.
GGC is dedicated to improving and integrating frailty pathways. Significant investment supports this initiative and the development of advanced frailty practitioner roles. The North Sector Older Peoples Service welcomes applications from experienced, innovative practitioners interested in service improvement and self-development, from across the NMAHP and pharmacy workforce.
These Band 7 advanced practice posts will operate across the primary/secondary interface, with practitioners aligned to Glasgow Royal Infirmary's front door/receiving area, maintaining close links with HSCP colleagues locally. Critical to success will be facilitating communication within multidisciplinary teams across acute and community services. Practitioners will work as a virtual network, enabling high-level joint clinical decision-making to ensure care is provided at the right time and place, maximizing outcomes, preventing avoidable admissions, and supporting timely discharges. An education and governance framework is in place, requiring all post holders to complete specific Masters-level qualifications. Applicants must be registered with the appropriate body and hold a full driver’s license.
