Job summary
A healthcare organization in Durham Region is seeking a Telehomecare Clinician to provide remote monitoring and coaching for patients with chronic diseases. The ideal candidate will have a degree in Nursing and experience in patient-centered care. You will be responsible for implementing care plans, monitoring patients, and delivering health education.
