Team Manager Keane Premier Support Services

JOB PURPOSE

As part of a multidisciplinary team, the post holder will have responsibility for assessment of care needs and the development of care plans & risk assessments, and the implementation and evaluation of these programmes ensuring the delivery of high-quality care and outcomes for service users.

The post holder is expected to carry out all relevant forms of care without direct supervision. The post holder may be required to demonstrate procedures to and supervise SSSC registered care staff.

The post holder will direct, coordinate and supervise, the work of other staff within their designated team to promote best practice in relation to People we Support.

SCOPE AND RANGE OF ROLE
  • Provide high quality, safe and supportive care to people we support within their own home, meeting identified physical, psychosocial and spiritual needs
  • Supervise and teach care staff, providing mentorship, guidance, induction and orientation to the service.
  • Ensure that your designated runs are effectively managed in partnership with the Support Workers.
  • Carry out all relevant forms of care without direct supervision.
  • Promoting self‑care, enabling people with long term conditions to live as independently as possible in their communities by ensuring that appropriate services and support are available to the People we Support and their family.
  • Facilitate guidance, mentorship and supervision of more junior staff within the service. Developing and maintaining the ethos of the learning organisation.
  • Responsible for assessment of care needs and the development of care plans and risk assessments, and the implementation, delegation and evaluation of these. Ensure that initial support plan is implemented with 4 weeks of receiving summary of needs.
  • Continually develop and maintain care plans, arrange and lead 6 monthly care reviews.
  • Ensuring the delivery of high‑quality outcome focused care for People we support.
  • May be required to demonstrate procedures to and supervise SSSC registered care staff and provide induction and orientation to the service.
  • Ensure that your allocated team/run has adequate supply of PPE.
  • Ensure that you manage the Rota for your team, to ensure effective use of resources.
  • Lead and support staff to ensure that Mandatory training levels are maintained.
  • Be a role model of the company and promote the company values in their actions.
MAIN DUTIES/RESPONSIBILITIES
  • Assess, plan, develop, implement and evaluate programmes of care for people we support to ensure delivery of a high standard of outcome focused care.
  • Maintain effective communications with people we support, relatives and other members of the multidisciplinary team to ensure that appropriate information is shared, and the person we support needs are met.

  • Ability to work using own initiative and maintain effective communications with People we Support, relatives and other members of the multidisciplinary /agency team to ensure that appropriate information is shared, and the person we support needs are met.
  • Responsible for the management of their team including reviewing and reporting absence effectively; performing well‑being checks on teams and offering support where available and the accurate recording of the reason for absence and discussions around this.
  • Responsible for rota management inline with service needs and contractual obligations to staff.
  • Leading by example showing exemplary conduct and behaviour in work, and supporting the team by reviewing instances of concerning behaviour; ensuring it is recorded and dealt with appropriately.
  • Engages the teams in their area to deliver the vision and live the values.
  • Challenges behaviour at all levels that does not fit with the values.
  • Demonstrates the organisations values in their daily behaviour.
  • Lead and supervise junior staff who are providing care and act as a source of advice and mentorship to ensure educational needs are met, including mandatory training, & induction of new team members.
  • Undertake teaching of SSSC registered care staff and participate in the implementation of staff personal development plans to facilitate ongoing development
  • Responsible for the immediate reporting of SSSC concerns and ensuring incidents and reports are documented in a detailed and timely manner.
  • Referrals to Health & Social Care Partnership following assessment of needs, including follow up on actions.
  • Required to perform risk assessments of staff when required, ensuring these are recorded and supplied and followed
  • Hearing initial concerns of team and managing and resolving these appropriately, ensuring an appropriate solution is achieved.
  • Supported by the HR department; have involvement with leading or participating in investigations, grievances and disciplinary actions; ensuring all relevant information and documents are completed in a timely and satisfactory manner while upholding the strictest of confidentiality.
  • Actively thanks and recognises staff for their achievements, growth and demonstration of the values of the company. Able to identify potential talent and recognises team actively and frequently.
  • To maintain a safe environment ensuring appropriate Moving & Handling is always used.
  • To undertake any other activities which may be deemed necessary by the Service or Deputy Service Manager.
Safeguarding of Vulnerable Adults
  • To complete training on Adult Support & Protection of Vulnerable Adults and be able to understand and implement ASP policies and procedures taking responsibility for reporting any safeguarding concerns to the senior person on duty or to the Home Manager.
