A healthcare organization in the United Kingdom is seeking a Social Work Resource Coordinator to deliver non-clinical social work services. This role involves coordinating resources, assisting patients and families in accessing community services, and collaborating with interdisciplinary teams. Candidates should possess a Bachelor’s Degree in Social Work and experience in a hospital or community health setting. Competitive compensation offered.
11a.m.-7:30p.m. with EOW rotation
The Social Work Resource Coordinator is responsible for the provision of non-clinical social work services, including resource referrals, assistance with behavioral health transfers, social determinants of health intervention, and administrative support. The Social Work Resource Coordinator functions as a member of the interdisciplinary care team providing social work expertise to assist with psychosocial barriers to care, as well as supporting throughput and readmission initiatives.
A Bachelor’s Degree in Social Work or related field. Experience in a hospital setting or experience working as a community health worker strongly preferred.
