We have an exciting opportunity for a committed worker, with experience of working with 16–25 year olds, to join our team as a Leaving Care Personal Adviser.

About You...

As a Leaving Care Personal Adviser, you will develop an effective working relationship with young people eligible for leaving care services. You will motivate and effectively support them to engage in education, training and employment whilst ensuring they effectively handle their finances, live in suitable housing, and have the necessary emotional resilience and skills for independence.

As a Leaving Care Personal Adviser, you will need a wide range of skills including knowledge of issues that impact care leavers and unaccompanied young people. You will play a key part in supporting them into adulthood and preparing them with skills to ensure they have the best chances moving forward independently.

You will need to think creatively and take the lead on some participation and group activities for the cohort of care leavers that we work with.

The Leaving Care Team is a small, dedicated team providing essential support to ensure we provide the best support to our vulnerable young people and care leavers.

We are a small, welcoming and friendly local authority. Key responsibilities...

You will be supported to carry out the following duties, responsibilities and training…

  • Carry out holistic assessments of young people leaving care.
  • To act as a personal adviser to a caseload of young people leaving care.
  • To promote the Leaving Care Service to partner agencies.
  • To work in partnership with other professionals.
  • To actively refer care leavers into appropriate services.
  • To maintain appropriate case management records.
  • To work flexibly to meet the needs of young people leaving care. This may include traveling out of area and working in the evening.
