Role: Advanced Clinical Practitioner

Location: Arrowe Park UTC, Wirral

Hours: 8am-6.30pm Mon-Fri (1-3 days)

Contract: Permanent

We're a small, not-for-profit organisation running a supportive, well-led Urgent Treatment Centre. We have 3 x 10 hour shifts available for ACPs with 5 years experience post-qualification.

Main duties of the job
  • See and assess patients presenting with urgent but non-life-threatening conditions (minor injuries, infections, acute illness, and exacerbations of long-term conditions)
  • Take comprehensive histories, conduct examinations, and formulate differential diagnoses
  • Use clinical judgement to manage risk and prioritise patients effectively
  • Provide timely and evidence-based treatment plans
  • Prescribe medications and interventions where appropriate
  • Arrange onward referral if needed
  • Communicate clearly with patients, carers, and multidisciplinary colleagues
  • Record consultations accurately and efficiently using EMIS &SystmOne
  • Ensure all documentation meets governance and medico-legal standards
  • Provide guidance and support to the clinical team as needed (e.g. nurses, or trainees)
  • Participate in clinical discussions, learning sessions and audits
  • Recognise and respond to safeguarding concerns in adults and children
  • Escalate concerns appropriately and follow local safeguarding protocols
  • Follow protocols and contribute to audit, feedback, and service development
  • Maintain registration and revalidation through CPD and professional development

Job responsibilities

The following are the core responsibilities of the Advanced Clinical Practitioner. This list is intended to summarise the key responsibilities and is not intended to cover every task that may be required for the role. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and colleague levels:

Clinical Knowledge and Skills

  • Assess and treat a wide range of patients, with undifferentiated, diagnosed and undiagnosed conditions, in a variety of settings and the patients own home, both face to face and by telephone consultation.
  • Undertake direct clinical management of patients, including history taking, systemic physical assessment, diagnosis, formulation of clinical management plan and prescribing appropriate treatment, in accordance with both national and organisational guidelines.
  • Manage complex and changing clinical situations including deteriorating patients, palliative and end of life patients and mental health presentations.
  • Prescribe and supply or administer medication and treatments according to own scope of clinical practice and organisational policies.
  • Participate in the OOH triage service if required. Accurately assess, prioritise and arrange review or referral of patients, if appropriate, using a variety of techniques to elicit and interpret history of an event or illness, including past medical history and drug history, in order to reach a clinical decision. Offer safe and evidence-based advice for self-care if appropriate.
  • Refer to and liaise with other agencies, including secondary care providers, community nursing teams, hospice colleagues as required.
  • Use excellent communications skills to ensure patients and their loved ones or carers gain the fullest understanding of their health needs and services available, including those who may be at risk because of barriers to communication.
  • Maintain accurate and contemporaneous records.
  • Act as an advocate for the patient.
  • Act as a role model for colleagues, providing best practice in all aspects of care, and ensure care is guided by agreed policies and procedures. This specifically includes practice relating to safeguarding and medicine management.
  • To work as part of a wider team to ensure that the care of people living with Frailty is based on relevant best practice guidance including, NSFs and NICE guidance and ICB policies.
  • Responsibility for PatientCare

    • Recognise and work within own competence and professional code of conduct as regulated by the HCPC.
    • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
    • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
    • Deliver care according to NSF, NICE guidelines and evidence-based care.
    • Participate in the maintenance of quality governance systems and processes across the organisation and its activities.
    • In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.

    Proactive Patient Care Specific (where required):

    • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients.
    • Clinically examine and assess patient needs from a physiological and psychological perspective, and develop person centred and holistic plans of clinical care accordingly.
    • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs seeking advice as appropriate.
    • Prescribe and review medication within personal ability for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
    • Work with patients in order to support compliance with and adherence to prescribed treatments.
    • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
    • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care and working with our multiple stakeholders to ensure best patient care, ensuring patients receive the right care at the right time.
    • Support patients and their families to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
    • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
    • Communication
    • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
    • Anticipate barriers to communication and take action to improve communication.
    • Utilise and demonstrate sensitive communication styles, to ensure patients and their families are fully informed and consent to treatment.
    • Maintain effective communication within the practice environment and with external stakeholders.
    • Act as an advocate for patients and colleagues.
    • Act as a representative of the organisation to ensure One Wirral CIC is held in high regard
    • Maintain up to date professional registration.

      Maintain clinical credibility by demonstrating continuous professional development.

      Maintain professional training and registration requirements for area of responsibility.

      Undertake training and education as required and ensure the highest standard of evidence based practice.

      Analytical Tasks

      • Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.
      • Review and process data using accurate SNOMED codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
      • Understand responsibility of self and others to the practice and primary care trust regarding the Freedom of Information Act.
      • Present clinical data and information to the MDT team to agree patient pathway.

      Planning and Organisational Skills

      • Implement agreed plans and input ideas for future service developments.
      • Plan and organise own duties so that they are completed accurately and within agreed time frames.
      • Reflect and monitor effectiveness and quality of service provisions and work closely with service managers to improve delivery.

      Physical Skills

      • Able to travel across various sites if needed.
      • Work is physically challenging and will include physical examination of patients, lifting and handling of patients and equipment.
      • Undertake a range of diagnostics including ECG and phlebotomy and near patient testing as appropriate.
      • Keyboard skills; knowledge of electronic care records and mobile working, databases, spreadsheet, email, word processing, presentation, email/Internet/Intranet and clinical software systems

      Policy and Service Development Implementation

      • Contribute where necessary to the development of service policies and guidelines for all, sharing ideas for service improvements within the team meetings.
      • Monitor own work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.
      • Advocate for policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all.
      • Duties may vary from time to time without changing the general character of the post or the level of responsibility.
