Job summary
A regional health service agency in Northeastern Ontario is seeking a full-time Care Coordinator (RN, PT, OT, SW). The role involves assessing patient needs, coordinating care delivery, and collaborating with patients and families to ensure patient-centered care. Candidates should have relevant regulatory membership and strong communication skills. This position offers an hourly wage of $41.29 to $51.13 and a defined benefit pension plan.
