We are looking for an experienced Practice Nurse to join our team. You will be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within your scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention as well as supporting the management team in reviewing clinical policy and procedure.

We are a member practice of the Blackburn with Darwen LPC Federation.
This is a 30‑hour position with the hours of work as stated: Monday 08:00–18:30, Thursday 09:30–20:00, Friday 08:00–18:30. You will be required to be flexible to cover the needs of the service.
Main duties of the job
- Develop, implement and embed health promotion and well‑being programmes.
- Implement and evaluate individual treatment plans for chronic disease patients.
- Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on the patients’ health.
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NHS.
- Request pathology services as necessary and process pathology results as required.
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
- Maintain accurate clinical records in conjunction with extant legislation and ensure read codes are used effectively.
- Maintain chronic disease registers.
- Develop, implement and embed well‑woman and well‑man clinics.
- Chaperone patients where necessary.
- Prioritise health issues and intervene appropriately.
- Recognise, assess and refer patients presenting with mental health needs.
- Implement vaccination programmes for adults and children.
- Support patients in the use of their prescribed medicines or over‑the‑counter medicines (within your scope of practice).
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
- Support the clinical team with all safeguarding matters, in accordance with local and national policies.
- Deliver opportunistic health promotion where appropriate.

Darwen Healthlink is a high QOF achiever and we use EMIS Web, LexCom, Remote Access and Iplato. Our patient list size is currently around 16,000 patients and we are located in a modern multipurpose health centre with a variety of community‑based healthcare providers within the building such as treatment rooms, dental, podiatry, MSK, audiology and an on‑site community pharmacy. The health centre is located just off junction 4 of the M65 providing easy access to the motorway networks.
Our practice team consists of 4 GP partners, 1 practice manager, 2 salaried GPs, 1 advanced nurse practitioner, 1 clinical pharmacist, 5 practice nurses and 1 healthcare assistant who are supported by an effective and professional administrative team who have been provided with training on workflow optimisation and care navigation.
Primary responsibilities
