Diabetic Practice Nurse

Location: Leicester
Salary: Depending on experience (Band 6–7 equivalent)
Hours: Full or part-time considered

About the Role

We are looking for an enthusiastic and experienced Practice Nurse with an interest in diabetes to join our friendly and supportive team in Leicester.

The successful candidate will play a key role in delivering high‑quality diabetes care to our patients, working closely with GPs, healthcare assistants, and the wider primary care network. You’ll be responsible for running diabetes clinics, providing education and advice, and helping patients to manage their condition effectively.

This is a great opportunity for a nurse who enjoys variety, values teamwork, and takes pride in delivering patient‑centred care.

Main Duties
  • Provide diabetes management and education through nurse‑led clinics.

  • Undertake annual reviews, blood monitoring, foot checks and lifestyle assessments.

  • Support medication reviews and insulin initiation where appropriate.

  • Offer advice on diet, glucose monitoring and long‑term condition management.

  • Maintain accurate records and contribute to QOF targets.

  • Participate in clinical meetings, audits and quality improvement work.

About You
  • Registered Nurse with current NMC registration.

  • Experience in diabetes care or chronic disease management.

  • Relevant training in diabetes management (or willingness to complete).

  • Independent Prescriber qualification is an advantage.

  • Good communication skills and a proactive, organised approach.

  • Comfortable working both independently and as part of a multidisciplinary team.
