A regional healthcare provider in Sunderland seeks a highly skilled Community Registered Nurse to support patients with long-term conditions. You will deliver compassionate and effective nursing care and educate patients on self-care strategies. The ideal candidate should have a valid NMC registration and a degree in nursing or be working towards it, along with excellent interpersonal and communication skills. This position involves both direct patient interaction and collaboration with a multidisciplinary team, ensuring comprehensive management of patient care.

The Steps to Health service in Sunderland provide an innovative and dynamic approach to individuals who are at risk or who have developed a long‑term condition and would benefit from exercise and rehabilitation to keep them well. Education complements a programme of rehabilitation to provide knowledge and support enabling patients to make healthy choices and self‑care.
We would like to invite applications for a Registered Nurse to join our team where you will play a key role working with specialists supporting assessments of patients referred to the service and initiating a plan of care.
You will be directly involved in education and in the delivery of exercise classes which can be face to face or virtual.
