Job description

JOB SUMMARY:

The Advanced Practice Leader (APL) is a crucial role that reports to the Director, Care Services and Education, within the Professional Practice and Operational Excellence Team. This position is instrumental in driving clinical excellence, innovation, and quality improvement within SE Health. The APL is responsible for advancing evidence-based practices, spearheading program, policy and education development, and fostering interdisciplinary collaboration to enhance patient outcomes. The position requires a highly skilled and experienced healthcare professional capable of navigating complex healthcare environments and driving positive change in client care delivery.

Key Responsibilities

  • Implement evidence-informed practices and provide expert clinical knowledge to improve care delivery for the target population.

  • Lead the development of innovative programs and care delivery models in collaboration with operations leadership and interdisciplinary teams.

  • Monitor and ensure compliance with federal, provincial, and other relevant legislation/regulations, accreditation requirements, developing strategies to address improvement opportunities.

  • Provide expert clinical knowledge and professional practice leadership to develop exceptional care solutions and service delivery models.

  • Collaborate with operations leadership to implement and sustain exemplary clinical practices.

  • Foster a culture of critical clinical inquiry and evidence-informed practice.

  • Contribute to quality, safety, and risk management initiatives, including the development of quality improvement plans.

  • Work towards improving client outcomes based on the best available evidence and efficient resource utilization.

  • Support Personal Support Educators, Clinical Practice Coaches, Managers, and the Clinical Practice Resource Team through regular meetings, orientation, mentorship, and guidance in complex clinical situations related to unregulated care provision.

  • Develop and implement educational initiatives, including support for innovative online learning environments.

  • Provide leadership in adult education, curriculum development, and delivery.

  • Participate in business development initiatives as required.

  • Advocate for policy and practice changes that align with emerging healthcare trends

  • Collaborate with multiple stakeholders to foster change and improvement in healthcare delivery.

  • Engage with external partners, academic institutions, and research bodies to advance clinical excellence

  • Leverage data to inform decision-making and drive innovative practices.

  • Apply research and analytical skills to evaluate data and integrate evidence into practice.

Qualifications and Skills

  • Education and Professional Registration

  • Nursing Diploma is required (minimum requirement)

  • Undergraduate degree in a health care related field is an asset

  • Current registration as a Regulated Health Professional in good standing with the College of Nurses of Ontario (CNO).

  • Additional certifications in leadership and/or quality improvement are an asset.

  • Experience and Expertise particularly in the field of unregulated practice and delegation

  • Minimum of 5 years clinical experience.

  • Demonstrated proficient clinical skills in areas such as delegation to unregulated care providers, medication management, wound care, risk assessment, analysis, risk mitigation etc.

  • Servant Leadership Mindset and Management Skills

  • Proven ability in program development, implementation, project change management, and evaluation.

  • Experience in quality improvement strategies and methodologies.

  • Demonstrated proficiency in knowledge translation and implementing evidence-based practices.

  • Superior interpersonal and communication skills, with the ability to direct and motivate teams.

  • Technical and Analytical Skills

  • Proficiency in Microsoft applications (Word, Excel, PowerPoint, and Outlook).

  • Ability to leverage data for decision-making and innovative practices.

  • Strong critical thinking, analytical, and problem-resolution skills.

Additional Requirements

  • SE Health highly values ongoing professional development. If the successful candidate has a baccalaureate degree, enrollment in a Master’s program may be required within 1-2 years of hire. We would be happy to support your continued education as part of your growth with our company. If the successful candidate does not have a baccalaureate degree, enrollment in an approved baccalaureate degree may be required.

  • Avalid driver’s license and access to a reliable vehicle required criminal background check and a vulnerable sector screen required

  • On-call support for the Clinical Practice Resource Team (CPRT).

This role is primarily virtual. You will be required to travel to home office in Markham, Ontario for 2 days every two months, at minimum. There may be other ad hoc travel expectations as required. A vehicle and ability to travel andsome on-call is required.
