Job description
  • Assist Pharmacy Manager to manage the day to day functioning of the pharmacy department
  • Offer advice on the prescription and use of medications to medical, nursing and pharmacy staff.
  • Dispense drugs to patients and provide advice on how to take them safety, counsel patients on appropriate dosing and routes of administration.
  • Supervise the prescriptions dispensed do patients to ensure their safety, quality, and compliance with applicable law.
Duties and Responsibilities
  1. Perform activities in the pharmacy area according to hospital polices and directives, Standard practices of the profession and pharmacy legislation.
  2. Monitor drug therapy as assigned by medical staff, suggest alternative drug therapy when appropriate.
  3. Supervise staff compounding, label and package medications and pharmaceuticals.
  4. Ensure medications are dispensed with clear instructions and correct labeling to patients.
  5. Check that drugs ordered to respond with conditions of patients, overdoses do not occur, allergies and drug interactions have been excluded.
  6. Check that medication orders entered dispensing program are accurate and utilized Care21 correctly.
  7. Ensure and provide dear, understandable instructions to discharge patients in a language known to patient, (if applicable).
  8. Comply with Pharmacy Legislation and maintain all records in the hospital as required by the Dangerous Drug Act 1952 and the Psychotropic Substances Regulations 1989.
  9. Consult and advice medical staff personnel regarding utilization of drugs and pharmaceuticals.
  10. Ensure secure storage of narcotics and other controlled substances on a regular basis.
  11. Develop interdisciplinary communication with nursing and medical staff. Co-operate with nursing to improve patient care through in-service teaching.
  12. Liaise with nursing and medical staff concerning prescriptions orders, drug and pharmaceutical a details, patients' reactions, errors and complaints.
  13. Investigate reports of incidents, medication errors and patient complaints. Resolve problems or makes recommendations to Pharmacy Manager for actions.
  14. Assist to communicate policy and procedure updates to pharmacy staff when necessary,
  15. Perform orientation of new staff.
  16. Supervise and train other pharmacy staff.
  17. Attend monthly staff meetings.
  18. Attend meetings and participates on committees as directed by Pharmacy Manager.
  19. Evaluate and review medications for addition to or deletion from the Formulary to Pharmacy
    Manages,
  20. Assist Pharmacy Manager to drug utilization reviews and prepare drug review criteria for medical staff review.
  21. Perform on call duty as scheduled
  22. Enhance professional growth and development through participation in education programs, current literature, in service meetings and workshops. Remain apprised of latest pharmaceutical
    products, developments, and laws
  23. Use pharmacy computer system to maintain records and track inventory. Assist in stock take and drug audits.
  24. Perform other related duties as arranged or requested.
Education and requirements
  • Graduate of a Ministry of Health recognized University of a College of Pharmacy, B. Pharm Degree/ B. Science (Pharmacy). Registration by the Pharmacy Board, Ministry of Health as Registered Pharmacist and possesses a valid Annual Retention Certificate Issued by the Pharmacy Board.
  • Shall be a registered pharmacist by the Pharmacy Division, Ministry of Health, Capable of obtaining a poison license to practice in the hospital pharmacy premises.
  • Shall have a minimum of four (4) years experience in hospital pharmacy practice.
  • Knowledge in purchasing and inventory control.
  • Good command of English and Bahasa Malaysia, both written and Bahasa Malaysia
