A healthcare organization is seeking an Adult and Children Safeguarding Care Coordinator to ensure effective safeguarding practices. This role involves maintaining accurate safeguarding lists, coordinating referrals, and supporting clinical teams to manage safeguarding concerns. The ideal candidate will have experience in the NHS and a commitment to safeguarding practices. This is a permanent position, requiring 20 hours per week, and is based across several surgeries in Gloucester.
The Adult and Children Safeguarding Care Coordinator plays a central role in supporting safe, effective and consistent safeguarding practice across the practice and PCN. The post holder will coordinate safeguarding processes for adults and children, ensuring safeguarding lists are accurate, actions are followed up, information is shared appropriately, and governance requirements are met.
The role supports Safeguarding Leads by maintaining oversight of safeguarding activity, coordinating meetings, audits and reporting, and ensuring compliance with local and national safeguarding guidance. The post holder will work closely with clinical teams, safeguarding leads, social care, and partner agencies to ensure safeguarding concerns are identified, recorded, escalated and reviewed in a timely and robust manner.
This is a non‑clinical but essential coordination and governance role within the multidisciplinary team.
Maintain accurate and up‑to‑date safeguarding registers and lists for both adults and children, ensuring regular review and follow‑up for safe practice.
Coordinate safeguarding processes.
Track and follow up new safeguarding referrals, including MARF, MARAC and MASH referrals, ensuring outcomes and actions are recorded and escalated appropriately.
Monitor safeguarding cases pending action, including awaiting MASH outcomes, case conference reviews or escalation decisions.
Coordinate regular safeguarding meetings, including preparing agendas, triaging cases prior to meetings, and ensuring all relevant internal and external attendees are informed and invited.
Record, disseminate and follow up actions from safeguarding meetings and reviews, ensuring accountability and timely completion.
Ensure safeguarding coding on clinical systems is accurate and aligned with local safeguarding guidance.
Standardise safeguarding reporting processes across the practice and PCN.
Support safeguarding audits and assurance work as required.
Ensure safe, accurate record keeping using SystmOne, including documentation of safeguarding discussions, meetings and reviews.
Support Safeguarding Leads by ensuring dedicated time is protected for complex case discussion, reflection and escalation.
The Safeguarding Coordinator is responsible for disseminating safeguarding information and delivering training to Safeguarding Administrators when processes are updated.

Job Description
Purpose of the role

Main Responsibilities

Adults subject to Section 42 safeguarding enquiries or plans

Support consistent, high‑quality information sharing in line with local safeguarding protocols and information governance requirements.

Ensure safe, accurate record keeping using SystmOne, including documentation of safeguarding discussions, meetings and reviews.
Support Safeguarding Leads by ensuring dedicated time is protected for complex case discussion, reflection and escalation.
The Safeguarding Coordinator is responsible for disseminating safeguarding information and delivering training to Safeguarding Administrators when processes are updated.
Key Tasks
1. Safeguarding List Management and Oversight
Maintain and regularly audit safeguarding lists, reviewing at least monthly (or more frequently when volumes increase).
Ensure safeguarding registers reflect current status and risk.
Proactively identify cases requiring follow‑up, escalation or review.
Flag safeguarding risks, delays or concerns to Safeguarding Leads.
2. Safeguarding Case Coordination
Receive and log safeguarding concerns raised by clinicians and staff.
Track referrals and outcomes from safeguarding teams and partner agencies.
Maintain clear safeguarding chronologies and case summaries.
Ensure safeguarding actions are followed up and documented.
3. Safeguarding Meetings and MDT Coordination
Coordinate regular safeguarding meetings.
Support triage of cases prior to meetings to prioritise risk and complexity.
Ensure appropriate internal and external attendees are informed and prepared.
Minute meetings accurately using SystmOne and agreed templates.
Track and chase actions following meetings.
4. Governance, Audit and Reporting
Support safeguarding audits and quality assurance work.
Ensure reporting is standardised and consistent.
Support preparation of safeguarding evidence for CQC inspections and internal reviews.
Assist in identifying learning themes and areas for improvement.
5. Information Governance and Coding
Ensure safeguarding coding is accurate and aligned with local guidance.
Support appropriate, lawful information sharing inline with safeguarding thresholds.
Maintain strict confidentiality and GDPR compliance at all times.
Additional Duties
In addition to the above duties you will also be expected to perform the below key activities in line with your job role:
Complete mandatory training in line with PCN policy and procedures
To participate in the staff appraisal process
To keep yourself updated on all matters relating to PCN policy
NHS Constitution

Infection Control
The post holder is accountable and responsible for the prevention of healthcare associated infections by complying with all Infection Prevention & Control policies and procedures in line with legislation (Health Act 2006; Code of Practice for the Prevention and Control of Healthcare Associated Infections).
Health and Safety
All employees must be aware of the responsibilities placed upon them under the Health and Safety at Work Act(1974) to ensure that the agreed safety procedures are carried out to provide asafe environment for employees and visitors.
GDPR
The General Data Protection Regulation requires you to ensure compliance with all relevant policies and those procedures relevant to the area of work. The PCN will always seek to process your personal data in accordance with its obligations and your rights.
Confidentiality
Your attention is drawn to the confidential nature of information collected and used throughout the NHS. The unauthorised use of disclosure of patient, staff or other personal information is a dismissible offence. The unauthorised disclosure of information could also result in a prosecution for an offence, or action for civil damages, under the General Data Protection Regulation. You are required to observe the strictest confidence regarding any Confidential Information relating to work of the PCN, its patients and its employees.
Risk Management
All staff working in, or for the PCN have a responsibility for participating in the risk management programme. All post‑holders have a responsibility to assess all risks to systems, processes and environment and contribute to the clinical and corporate governance agendas as appropriate.
Safeguarding Duty
It is the responsibility of the postholder to be aware of and follow the legislation and guidance regarding Safeguarding Children and Adults. The post holder is responsible for ensuring they receive the appropriate level of Safeguarding training according to their role.
Information Technology
It is the responsibility of the postholder to have a level of IT competence relevant to their job role and will be expected to continue to keep their skills up to date as part of their Continuing Professional Development.
