A healthcare organization based in the United Kingdom is seeking a Care Coordinator to help manage and coordinate health and social care services for patients. The ideal candidate will have experience in health and social care, a GCSE level education, and strong IT skills, specifically with EMIS. This part-time role requires 15 hours of work per week, providing a fantastic opportunity to support individuals in their care journey. This vacancy is for 15 hours per week [Monday to Friday; 08:00 - 11:00].
Based in a single GP practice, the care coordinator will be involved in supporting the clinical team to proactively identify and work with patients, including the frail/elderly and those with long‑term conditions, to provide proactive, person‑centred care planning, helping coordinate care, by bringing together the different specialists whose help that individual might need. This might involve a wide range of services, such as hospital care, community care, social care, housing and the voluntary sector.

The site is a modern, purpose‑built premises, containing a variety of primary care services to meet the needs of the local population. We are proud to be a teaching practice for medical students, nursing students, health care assistants and GP trainees. We are committed to the continued development of our teams.
Care coordinators provide extra time, capacity and expertise to ensure that appropriate support is made available to patients and that their needs are addressed. They will help patients and their families understand, manage and utilise personal care and support plans and contribute to increasing the number of patients with personal health budgets.
Care Coordinators will work closely with Primary Care Network (PCN) and GP Practice to ensure patients receive coordinated care, which is responsive to their needs and designed in cooperation with patients, carers, and all supporting services.
