We have an exciting job opportunity for a Practice Nurseto join our large nursing team.

They will be responsible for the implementation ofprocesses for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.

Working as a senior member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population.

The Practice Nurse will be responsible for a number ofclinical areas such as infection prevention and control, health promotion,chronic disease management, well women and well man clinics, as well asactively supporting the practice management team in the reviewing and deliveryof clinical policy and procedure.

They will also carry our routine treatments in generalclinic such as, Wound Management, ECGs, Smears, Screenings and Imms.

At The Oaks Medical Centre we have 64 members of staffwhich include GP's,Nurses, Paramedics, Physicians Associates, Management Team,Receptionist, Prescribers, Data and Admin team and Secretary teams.

We are also part of KEN PCN, meaning we also have PCNstaff working here such as, physician associates, care coordinators etc.

To provide clinical nursing service and support to otherprofessionals and to patients of the practice including treatment, screening,chronic disease management/QOF and advice within the professional competence ofthe post holder. Support and work effectively within the clinical team.

All duties will be carried out in accordance with the NMCCode of Professional Conduct and with regard to the various practice protocols.The nurse is part of a nursing team responsible for ensuring that the practiceprovides an optimum service of nursing care relevant to the practicepopulation.

• Registered Nurse
• CPR trained
• Post graduate diploma or degree (chronic disease management)
• Family planning qualification
• Nurse prescriber

• Wound care/removal of sutures and staples
• ECGs
• Venepuncture
• Imms
• Chronic Disease Management
• Ability to follow and have knowledge of Clinical Procedures and Policy
• Knowledge of SystemOne and Read Coding
• Strong IT Skills
