Community Psychiatric Nurse/Care Co-Ordinator role at Black Country Healthcare NHS Foundation Trust. The closing date is 07 October 2025.

Following recent investment to support the transformation of community mental health nursing, we are recruiting additional Band 6 Community Psychiatric Nurses within the Community Recovery Services across the Sandwell locality. This is an opportunity to join a supportive, diverse, multi-agency team investing in transforming community mental health services across Black Country, with a new model focused on people living in their communities with mental health challenges.

As a Registered Mental Health Nurse, preferably with adult and community mental health experience, you will be responsible for assessing and formulating individual interventions, plans and outcome measures that promote recovery, in line with the recovery model and relevant legislation for all people on your caseload. You will manage a caseload of service users referred to the team via multiple access points, be involved in crisis management as part of the duty system, and undertake the role of the duty nurse on a rostered basis. You will work from our community base facility, via phone/agile working or within the service user\'s home. You will need up-to-date knowledge of NHS policies and practices in mental health, have experience of a range of therapeutic interventions and psychotropic medications, and have proven ability to work with partner agencies, lead projects and motivate others. You will have excellent communication skills with a flexible approach. The team offers regular supervision and opportunities to develop clinical and professional practice in line with service users\' needs and organisational values.

Everyone who lives in the Black Country will have fair access to mental health services that are sensitive to the needs of our Dudley, Sandwell, Walsall and Wolverhampton communities. Access will be via a single point of access. Service users will have a named mental health worker who will remain the key contact and help access the right services and support (health, care and voluntary support). Service users will receive personalised care from a range of staff with different skills and experiences, working together as one team.

• RMN with post-registration experience in multidisciplinary community mental health services.
• Excellent written and verbal communication skills with internal and external stakeholders (staff, patients, carers, external agencies).
• Specialist skills in assessing and formulating intervention plans for adults with mental health needs.
• Ability to work independently and as part of a team; strong initiative and IT/administrative skills for accurate record keeping.
• Experience of collaborative working across agencies and professional disciplines; strong risk assessment and management skills.
• Knowledge of current government directives, legislation (including MHA 1983) and safeguarding.
• Experience with psychotropic medications and psychosocial interventions; ability to manage workload and competing demands.
• Flexibility of working hours and a commitment to recovery, social inclusion and person-centered care.
• Ability to lead and motivate projects and people; willingness to commute to meet post requirements.
