Overview

Main responsibilities

The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way, helping to prevent longer term challenges and admissions to hospital.

Your role will involve:

  • Assessments and interventions for residents with physical disabilities, not excluding cognitive decline
  • Working with other professionals to support identified outcomes
  • Holding and managing a case load

Qualifications
  • Skills:
  • Good communication skills including written and spoken English.
  • Good interpersonal skills.
  • Good time management and organizational skills.
  • Self-awareness of own stress levels and how to manage these.
Experience
  • Abilities:
  • Ability to adapt to changes in the workplace
  • Ability to work as part of a team
  • Ability to apply training to practice
  • Confidence in working autonomously and knowing when to seek support.
  • Ability to represent the Authority, at a multi-agency level, in planning and reviewing services for individuals.
  • Ability to problem solve.
  • Basic awareness of relevant legislation.
  • Able to express verbal information clearly and concisely.
  • Written information clear and concise.
  • Takes a proactive approach to meeting the needs of the person.
