The Older People’s Services Department forms part of the Medical Care Group comprising the following Directorates: Older People’s Services and Acute Medicine, Medical Specialities, Networked Medicine and Emergency Services.
The Directorate is led by a Clinical Lead, General Manager and Matron and includes all 10 Older People’s Services wards and The Day Hospital which is based out of Christchurch Hospital. Currently it has 34 consultants/specialists who are supported by a large team of junior doctors, Nurse practitioners and AHPs.
We believe our patients should be seen and assessed by a Geriatrician and the multi-disciplinary team within hours of arriving to the hospital. Our Frailty pathway enables us to identify frailty at the front door and provide the resource to get our patients home sooner. This means admissions from GP referrals and the Emergency Department come straight to our Older People’s admissions units. Once assessed, our patients will then move through our pathway to either short stay or complex care wards to continue their plan of care. Working together across the Older Persons Department is key to developing this vision. Our inpatient care facilitates safe discharge and is crucial to flow throughout the hospital.
Our values define who we are as #TeamUHD. They underpin everything we do now and in the future. They define how we treat our patients, visitors, and each other, our valued teammates and colleagues.
UHD are investing in services across the Trust, with many being transformed and developed following merger and the New Hospital Programme.
This means that some services may move site this year or next, either temporarily or long term. Recruiting Managers interviewing for this role will be happy to answer any specific questions at interview.
Please note if a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under relevant Trust or national terms and conditions.
