A healthcare service provider in Ellesmere Port is seeking a Band 6 Registered Nurse for the Hospital at Home service. The role involves providing urgent community response and virtual ward care for patients in their homes, alongside a multidisciplinary team. This permanent position requires 30 hours per week across a 7-day service with an emphasis on leadership, clinical care, and person-centred support. Competitive applicants must hold a valid nursing qualification and possess strong communication skills.
We are looking to recruit a Band 6 Registered Nurse for the Hospital at Home service, providing urgent community response and virtual ward care to West Cheshire residents in their own home. You will be joining a multidisciplinary team including but not limited to GP's, Advanced Clinical Practitioner, nurses, therapists, pharmacists and clerical officers.
We are looking for someone who is enthusiastic, self‑motivated, provides confident support and leadership and coaching to clients and clinicians. You must have excellent communication skills and an ability to work in partnership with clients and their families/carers and, when necessary, act to safeguard individuals who are at risk from harm and abuse.

This is a permanent post working 30 hours per week across a 7‑day service, between 8am and 8pm.
