Main duties of the job
- Deliver high quality, compassionate care under the direction of the Registered Nursing team, working as part of a multidisciplinary team to support patients with palliative and respite care needs.
- This role is for 37.5 hours per week; part-time hours may be considered.

Job Purpose: To monitor, contribute and inform high-quality palliative and end-of-life care to patients and families.
Responsibility Areas: It is expected the Nursing Associates will be able to carry out all core competencies outlined in the job description; duties and competencies may vary depending on area of work and extended competencies.
- Deliver high quality, compassionate care under the direction of a Registered Nurse with a focus on promoting patient health and independence.
- Work as part of a designated clinical care team delivering care that focuses on the direct needs of the patient throughout their lifespan.
- Clinical responsibilities – Individuals care: Abide by the principles of the NMC code of conduct and work within clearly defined accountability frameworks, recognising boundaries of the Nursing Associate role.
- Develop understanding of all elements of the nursing process through guided participation in assessment, planning, management and evaluation of care with increasing knowledge and competence.
- Support individuals with all aspects of care including personal care, promoting wellbeing, activities of daily living and person-centred care.
- Work with the multidisciplinary team to raise awareness and navigate care to other professionals as appropriate.
- Measure, monitor and record observations (e.g., TPR, BP, oxygen saturation, peak flow, ECG, urine tests, blood glucose, fluids).
- As prescribed, administer medications in a safe and timely manner in line with medicines management policies and training.
- Ensure privacy, dignity and safety of individuals at all times.
- Recognise changing priorities and seek advice from the Registered Nurse or other registered care professionals as appropriate.
- Report back to and share information with the Registered Nurses on the condition, behaviour, activity and responses of individuals.
- Contribute to the implementation of actions to meet physical, emotional, psychological, social, cultural and spiritual needs of individuals and carers.
- Contribute to the delivery of complex care as prescribed by the Registered Nurse and to the monitoring of clinical standards and outcomes.
- Demonstrate understanding of consent and ensure valid consent is obtained prior to procedures; understand the Mental Capacity Act / Deprivation of Liberty safeguards and seek guidance as required.
- Care Provision: contribute to holistic assessment, planning, implementation and evaluation of patient needs on each shift.
- Deliver general nursing care interventions responsive to individual patient requirements; report concerns or changes to the patient’s condition or family wellbeing to wider community teams as appropriate.
- Contribute to patient-centred assessment and care planning, utilise outcome measures (OACC) and modify care plans as required.
- Identify pre- and post-bereavement needs and refer to supportive care teams as needed.
- Support Senior HCAs during joint visits and act as a link nurse in selected specialisms (e.g., diabetes).
- Follow hospice and NHS clinical procedures aligned with NMC regulations for medicine management; follow policies for manual handling, infection control, health and safety, ensuring a safe environment.
- Planning: manage and prioritise own workload; undertake risk assessments and document them; escalate to senior colleagues as appropriate.
- Contribute to timely patient discharge planning from admission.
- Communication: liaise with relatives and carers regarding patient status; provide accurate written and verbal reports during handovers; encourage patients and families to express needs to inform care planning; promote collaborative and effective multidisciplinary team communication; ensure the nursing voice is represented in ethical decision making; respond to complaints per hospice policy.
- Promote safeguarding awareness for both children and adults and report concerns to appropriate professionals.
- Professional Leadership: act as a role model, support volunteers, develop coaching/teaching skills, and participate in formal and informal supervision and mentorship; assist in supervision and training of HCAs, student nurses and trainee nursing associates; deliver training sessions as required.
- Quality, Improvement, Audit and Research: contribute to audits and evaluations of palliative/end-of-life care; participate in reflective reviews; ensure clinical incidents/near misses are recorded and actioned to minimise risk.
- Health and Safety: maintain safe environments, ensure equipment is in good working order, manage waste and hazardous substances per COSHH; ensure infection control practices are followed.
- Education: maintain clinical competence through ongoing development, mandatory training, peer supervision, reflective practice and clinical supervision; maintain a professional portfolio and reflective diary.

- Nursing Associate registered with NMC
- Must be in possession of a full UK driving licence, be over 21 and have held a full licence for 12 months
