A regional healthcare provider is seeking an experienced Band 3 HCA to join their community team in Sandbach. In this role, you will deliver essential nursing interventions, support a multi-disciplinary team, and provide compassionate care for patients approaching the end of life. A strong commitment to empathetic relationships and excellent communication skills are required. This position offers ongoing training and support, with travel required across the Central Cheshire Integrated Care Partnership.

We are looking for an experienced Band 3 HCA to work within a busy community team. The post is based in Sandbach, working within SMASH care community.
The role includes caring for patients who are approaching the end of life. The post holder will undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency.
You will be working without close supervision, and will therefore need to use your own initiative, whilst following the patient's care plan. You must also be passionate about making a difference to people’s lives, helping them to live as well as possible and to die with dignity.
As well as being able to work independently, you will possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas – if required – to meet service need.
To join our dedicated, caring team, you will need excellent communication skills and emotional strength. We will expect you to have a talent for forging strong empathetic relationships with a variety of people, whilst providing palliative care. We will of course provide excellent on‑going training and support.
Your role will include the responsibilities listed above, and may require you to work across the footprint of Central Cheshire Integrated Care Partnership.
