This position with the Providence Care – Transitions Home Program will be engaged in community work and involves commuting across the southeastern Ontario region. The Transitions Home Program works at achieving timely access to care and clients’ functional outcomes following discharge from PCH/PTCC. Therapeutic interventions will be carried out in the client’s home and community.
The Social Worker, reporting to the Clinical Program Manager, acts as the fundamental resource to program staff, clients, caregivers and community partners. In collaboration with the program staff, the Social Worker provides psychosocial assessment, counselling/psychotherapy, case management, patient/family education, supportive counselling, discharge planning, service coordination and consultation according to organizational and best practice standards in order to assist patients to achieve optimum recovery and quality of life.
This includes maximizing the benefit patients and families receive from their medical treatments and transitioning to timely and risk-reduced discharges. The Social Worker provides leadership in high risk social situations, program development, advocacy, and developing and maintaining strong community linkages. The Social Worker aligns their work with the operational priorities related to access to care and transitions and is accountable for maintaining the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers.
As a member of the clinical team, the Social Worker participates in discipline-specific and inter-professional activities that are directed towards a client-centered environment and that contribute to quality, operational efficiency, professional and program development.
