Job description
Duties and Responsibilities
  • Assist Pharmacy Manager to manage day-to-day functioning of the pharmacy department.
  • Offer advice on prescription and use of medications to medical, nursing and pharmacy staff.
  • Dispense drugs to patients and counsel on appropriate dosing, routes, and safety.
  • Supervise prescriptions to ensure patient safety, quality, and compliance with law.
  • Perform activities in pharmacy area in accordance with hospital policies, directives, and standards.
  • Monitor drug therapy as assigned by medical staff; suggest alternative therapy when appropriate.
  • Supervise staff compounding, labeling, and packaging of medications and pharmaceuticals.
  • Ensure medications are dispensed with correct labeling and clear instructions to patients.
  • Check that drug orders respond to patients' conditions, prevent overdoses, exclude allergies and drug interactions.
  • Check that medication orders entered into dispensing program are accurate and utilize Care21 correctly.
  • Ensure discharge instructions are understandable and provided in patient’s language.
  • Comply with pharmacy legislation and maintain records required by Dangerous Drug Act 1952 and Psychotropic Substances Regulations 1989.
  • Consult and advise medical staff regarding utilization of drugs and pharmaceuticals.
  • Secure storage of narcotics and other controlled substances on a regular basis.
  • Develop interdisciplinary communication with nursing and medical staff; cooperate with nursing to improve patient care through in-service teaching.
  • Liaise with nursing and medical staff regarding prescription orders, drug details, patients' reactions, errors and complaints.
  • Investigate reports of incidents, medication errors and patient complaints; resolve problems or recommend to Pharmacy Manager for action.
  • Assist in communicating policy and procedure updates to pharmacy staff when necessary.
  • Perform orientation of new staff; supervise and train other pharmacy staff.
  • Attend monthly staff meetings; attend meetings and participate on committees as directed by Pharmacy Manager.
  • Evaluate and review medications for addition to or deletion from the formulary.
  • Assist Pharmacy Manager with drug utilization reviews and prepare drug review criteria for medical staff review.
  • Perform on-call duty as scheduled.
  • Enhance professional growth through participation in education programs, current literature, in-service meetings, workshops; remain apprised of latest pharmaceutical products, developments, and laws.
  • Use pharmacy computer system to maintain records and track inventory; assist in stock take and drug audits.
  • Perform other related duties as arranged or requested.
Education and Requirements
  • Graduate of a Ministry of Health recognized University or College of Pharmacy; B. Pharm Degree or B. Science (Pharmacy).
  • Registration by the Pharmacy Board, Ministry of Health as Registered Pharmacist with valid Annual Retention Certificate issued by the Pharmacy Board.
  • Registered pharmacist by the Pharmacy Division, Ministry of Health, capable of obtaining a poison license to practice in hospital pharmacy premises.
  • Minimum of four (4) years experience in hospital pharmacy practice.
  • Knowledge in purchasing and inventory control.
  • Good command of English and Bahasa Malaysia, both written and spoken.
