Job Summary: An excellent opportunity has arisen for a Band 6 Mental Health Practitioner to join the South West Living Well Centre in Streatham. As part of the Lambeth South West Focussed Support Team, you will play a vital role within a Community Mental Health Team (CMHT) providing care and treatment to individuals with severe and enduring mental health conditions. • Manage and coordinate care for a caseload of patients with complex mental health needs.
• Develop and implement personalised care plans to promote patient recovery and well-being.
• Conduct comprehensive risk assessments and apply effective risk‑management strategies.
• Work collaboratively with a multidisciplinary team to deliver holistic mental health support.
• Contribute to regular patient reviews, case discussions, and service improvement initiatives.
• Provide information, advice, and emotional support to patients and their families.

• Experience in care coordination and risk management within mental health services.
• Working knowledge of ePJS (Electronic Patient Journey System).
• HCPC registration or relevant professional qualification.
