Working Hours: Monday to Friday, 9:00am – 5:00pm

Job Summary: An excellent opportunity has arisen for a Band 6 Mental Health Practitioner to join the South West Living Well Centre in Streatham. As part of the Lambeth South West Focussed Support Team, you will play a vital role within a Community Mental Health Team (CMHT) providing care and treatment to individuals with severe and enduring mental health conditions. Key Responsibilities
  • Manage and coordinate care for a caseload of patients with complex mental health needs.
  • Develop and implement personalised care plans to promote patient recovery and well-being.
