Job Summary

CLCH Hertfordshire Adult Community Services – CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Are you an experienced Paramedic tired of the queue at A&E? Are you looking for a role where you have the time, resources, and autonomy to effectively see and treat patients in their own homes?

Do you want to bring your nursing expertise to the front line of crisis care? Use your advanced skills in long‑term condition management to assess patients at home, prevent hospital admissions and deliver true holistic care when it is needed most.

We are seeking enthusiastic Paramedics or Specialist Nurses to lead our established Early Intervention Vehicle (EIV) Service.

Working within the Unplanned Care Team, your primary goal is crisis management and admission avoidance. You will be the clinical lead on the ground, responding to non‑life‑threatening priority calls across the Herts valleys.

You will work side‑by‑side with a Therapist, Social Worker or Community Care Officer (CCO). Together, you will perform holistic physical and functional assessments, issue urgent equipment and put immediate care plans in place to keep the patient safe at home.

Main Duties of the Job
  • True multidisciplinary working: You aren't working solo. You will assess jointly with therapists to tackle both the medical and functional crisis.
  • Advanced clinical decision making: You will use your advanced skills to make complex clinical decisions quickly and autonomously. You will have the discretion to manage risk and support patients in crisis.
  • Direct impact: You will directly reduce unnecessary hospital admissions and facilitate early discharges (discharge to assess).
Key Qualifications
  • Registered Paramedic with HCPC registration or UK NMC registered Specialist Nurse.
  • Advanced clinical assessment skills.
  • Confidence to make autonomous decisions in a community setting.
  • Experience in urgent care, community care or the management of long‑term conditions.
  • A driving ambition to develop the service and strategic skills.
Experience
  • Recent substantial post‑registration professional clinical experience.
  • Extensive range of clinical skills and experience including IV therapy, venepuncture, cannulation and rehabilitation.
  • Experience of working within a multi‑disciplinary team.
  • Experience of supervising junior staff and coordinating a team on a day‑to‑day basis.
  • Experience of undertaking comprehensive health needs assessment and writing care plans.
Skills and Knowledge
  • High level of competence in a range of clinical skills.
  • Ability to work in a team.
  • Ability to work without direct supervision, planning and prioritising own workload and that of others.
  • Ability to delegate work effectively and safely.
  • Ability to work under pressure.
  • Ability to risk assess a situation and act appropriately.
  • Ability to communicate clearly and effectively, in both verbal and written formats.
  • Ability to identify own and others learning and development needs and actively seek ways to meet those needs.
  • Basic IT skills including use of Microsoft Office packages and email.
  • Knowledge of principles of care for patients with long term conditions, relevant national health standards.
  • Understanding of NMC / HCPC Codes of Professional Conduct and application to practice.
  • Personal development planning.
  • Primary health care structures and roles (overview).
  • The audit process.
  • Applying evidence‑based care into practice.
  • Principles of clinical governance and application to practice.
  • Clinical supervision – purpose and application.

Other Essential
  • Hold a full, valid UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

  • Ability to provide evidence of where you have demonstrated the Trust's values and behaviours.
