Job Title: Band 6 Community Psychiatric Nurse – Short Term Support

• Conduct detailed assessments and develop treatment plans to support individuals with low to moderate needs.
• Deliver effective psychosocial interventions within a 12-week support framework.
• Engage in duty work and coordinate with colleagues to manage discharge processes and reduce caseloads.
• Collaborate closely with a team of clinicians, social workers, and support workers to deliver high-quality short-term support.
• Ensure effective communication with patients, families, and healthcare professionals to facilitate comprehensive care.

• Degree or equivalent in Mental Health Nursing.
• Experience working as a Mental Health Nurse.
• NMC Registration required.
• Strong assessment and formulation skills are essential for this role.
