A healthcare organization in Chichester is seeking a Community Nurse to provide a triage and information service to patients. The role involves working collaboratively with service users with complex health presentations, as well as providing clinical supervision to staff. Candidates must have a valid nursing registration and experience working in community settings.
Although we are remote/working from home we do meet once a month in person as well as daily meetings over Teams. Ideally you will be located in the West Sussex locality.
This community nurse role will provide a triage and information service to patients referred to SOAMHS in West Sussex. The post holder will be part of the team that screens and determines the clinical pathway for all patients referred to the service. The majority of referrals will be made via the electronic referral system. The role is to work in partnership with service users in the designated care group with complex health presentations, demonstrating an understanding of clinical interventions relevant to the care group including education, signposting and pathway planning. The post holder will work at all times to promote the safety and well-being of service users and their families/carers. You will provide clinical supervision to staff and students.

Salary will normally be offered at the bottom of the relevant salary range, with consideration for previous NHS experience where demonstrated.
