Although we are remote/working from home we do meet once a month in person as well as daily meetings over Teams. Ideally you will be located in the West Sussex locality.

This community nurse role will provide a triage and information service to patients referred to SOAMHS in West Sussex. The post holder will be part of the team that screens and determines the clinical pathway for all patients referred to the service. The majority of referrals will be made via the electronic referral system. The role is to work in partnership with service users in the designated care group with complex health presentations, demonstrating an understanding of clinical interventions relevant to the care group including education, signposting and pathway planning. The post holder will work at all times to promote the safety and well-being of service users and their families/carers. You will provide clinical supervision to staff and students.

• Screen and determine the clinical pathway for all patients referred to the service.
• Provide a triage and information service to patients referred to SOAMHS in West Sussex.
• Work in partnership with service users, carers, families and partner agencies to facilitate pathway planning and education.
• Ensure a consistent approach to triaging and processing referrals, with majority referrals via the electronic referral system.
• Demonstrate excellent communication skills and provide clinical supervision to staff and students.
• Work within the community with complex health presentations and apply appropriate clinical interventions.

• Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role.
• Completion of Mentorship Course/ENB equivalent.
• Evidence of continuous professional development.
• Significant post-qualifying experience in the relevant care group.
• Experience of working alongside people with complex health problems in the designated care group.
• Experience of supervising staff and providing a range of clinical interventions to people in the designated care group with varied health problems.
• Experience of working consultatively with professionals and conducting clinical assessments including risk assessments.
• Experience of partnership working within and across statutory and non-statutory teams and agencies; understanding of the CPA process and clinical governance.
• Previous community-based experience.
• Disclosure and Barring Service (DBS) check. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975; a disclosure submission is required.
• Appropriate UK professional registration.
