About the Role

As a Community Nurse, you will be part of a dynamic and compassionate team delivering high-quality, evidence-based care to patients in their own homes, residential care settings, and clinics. You’ll play a crucial role in preventing unnecessary hospital admissions, promoting health and independence, and improving overall well-being in the community.

Responsibilities
  • Clinical Duties:
    • Deliver patient-centred care in homes, clinics, and residential facilities
    • Assess, plan, implement, and evaluate care packages tailored to individual needs
    • Promote independence, manage long-term conditions, and support early discharge
    • Perform clinical procedures including wound care, catheter care, and tissue viability assessments
    • Maintain accurate, up-to-date patient records in line with NMC standards
    • Provide emotional support and health education to patients and carers
  • Professional Practice:
    • Work within the NMC Code of Conduct and Trust guidelines
    • Collaborate effectively with multidisciplinary teams and external agencies
    • Support and supervise junior staff and contribute to their development
    • Participate in audits, quality improvement initiatives, and service development
  • Operational Duties:
    • Safely transport and manage clinical equipment
    • Use clinical systems and IT tools effectively
    • Submit travel and mileage claims timely
    • Comply with all mandatory training and health & safety procedures
  • Decision Making & Communication:
    • Make informed clinical judgments and onward referrals
    • Provide advice and guidance to patients, carers, and professionals
    • Deliver sensitive information with empathy and professionalism
    • Participate in case discussions and contribute to team meetings
What We're Looking For
  • NMC Registered Nurse (Valid PIN)
  • Minimum 6 months of NHS experience in the UK
  • Confident in clinical skills with excellent decision-making abilities
  • Strong communication and interpersonal skills
  • Ability to work autonomously and within a team
  • Flexible, reliable, and holds a valid UK driving license
Pay Rate
  • Days : £21
  • Night/ Saturday: £27
  • Sunday/ BH: £32
