We are recruiting a band 6 mental health nurse for the Specialist Older Adults' Mental Health Service (SOAMHS), based at Horsham Mental Health Team, Linwood Community Mental Health Centre,Haywards Heath. The Specialist Older Adults Mental Health Service is integrated between health and social care, and works with older people with both functional and complex organic conditions who need secondary service support. Experience of working with this client group in a community setting is advantageous but not essential, as we would offer a robust induction and supervision package for the right candidate.

This post offers the Trust's £2,000 Golden Helloor Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).

The ability to travel is essential for this role and must be willing to travel across Mid Sussex and including Horsham and Crawley and surrounding areas as required.

We will be looking for someone with a passion for working with older people. You will work as part of a multidisciplinary team and provide specialist assessment, treatment and care to a caseload of people with a range of mental health needs. You must be enthusiastic, self-motivated, flexible and resourceful and be able to forge close links with clients and carers. We also work closely with partner healthcare organisations and the voluntary sector.

The service operates 7 days a week including bank holidays.

The role of Urgent Care Practitioner is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.

• Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
• Completion of Mentorship Course/ENB equivalent
• Evidence of continuous professional development

• Significant post qualifying experience in the relevant care group
• Experience of working alongside people with complex health problems in the designated care group
• Experience of supervising staff
• Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
• Experience of working consultatively with professionals
• Experience of conducting clinical assessments including risk assessments
• Experience of partnership working within and across various statutory and non statutory teams and agencies
• Experience of working within the CPA process
• Previous experience of working in the community
