The Health & Care Outcomes team plays a central role in delivering safe, person-centred services across the borough.

The Role

We are recruiting one Adults Advanced Practitioner to provide long-term sickness cover within Adult Social Care. This hybrid role requires three days in the office and two days remote working. The Advanced Practitioner will take a lead role in safeguarding, adult protection, and managing high-risk, complex cases. You will work alongside practitioners, chair operational meetings, and ensure that interventions and care plans are person-centred, legally compliant, and outcome-focused.

Key Responsibilities
  • Manage complex caseloads involving safeguarding, adult protection, and high-level risk.
  • Work alongside practitioners on cases requiring critical decision-making and court involvement.
  • Chair adult protection operational meetings where risk and complexity are high.
  • Ensure timely assessments, support planning, and reviews with individuals and carers.
  • Authorise and quality assure reports, assessments, and court documentation.
  • Support and mentor practitioners, promoting best practice and professional standards.
  • Contribute to service development projects and safeguarding interventions.
  • Respond to Councillor queries and attend Committee meetings when required.
  • Authorise resource commitments within delegated levels.
Candidate Requirements
  • Qualified Social Worker (Degree in Social Work, DipSW or equivalent).
  • Registered with Social Work England.
  • Evidence of continuing professional development in line with the Professional Capabilities Framework.
  • Substantial post-qualification experience in statutory health or social care settings.
  • In-depth knowledge of safeguarding, mental capacity, and relevant legislation.
  • Strong assessment, interviewing, and risk management skills.
  • Experience supervising, mentoring, and supporting practitioners.
  • Ability to chair meetings and manage multi‑agency partnerships effectively.
  • Excellent communication skills, both verbal and written.
  • Commitment to anti‑discriminatory practice and equal opportunities.
Desirable
  • Recognised management qualification.
  • Experience working with a wide range of families with diverse needs.
