Job description

We are looking for passionate and skilled Band 6 Community Nurses to join our Thanet Community Nursing Teams, based at our brand new £10million state‑of‑the‑art Thanet Health Hub.

Our award‑winning service brings together a skilled group of nurses, physiotherapists, exercise physiologists, counsellors, cardiac rehab assistants, practitioners, an occupational therapist, administrators and volunteers.

We provide physical, social and psychological rehabilitation through a variety of options tailored to each patient’s needs and preferences.

In this role, you’ll deliver cardiac rehab services across an allocated area of East Kent and help cover other areas based on demand, waiting times and capacity.

You’ll also work closely with the Senior Clinical Lead and Head of Service to ensure we deliver safe, high‑quality care through an effective and efficient model.

You’ll be responsible for assessing, delivering and evaluating care for patients with complex, long‑term and potentially high‑risk cardiac conditions. As part of a multi‑disciplinary team, you’ll manage a set caseload of patients with ongoing needs who would benefit from targeted case management.

Working within the community, you’ll serve as a go‑to resource on Coronary Heart Disease for primary, community and secondary care services.

Additional Working Pattern: 22.5 Monday‑Friday
