Program: Supportive Housing - Community Supports & Outreach
The WoodGreen Community Supports and Outreach (CSO) program serves clients 18 years and above living with mental health challenges and addictions, requiring a supportive housing environment. The program offers individualized case management, community engagement, and pest control & hoarding management across small-scale apartment buildings in East York and Downtown Toronto, as well as in scattered independent units with rent supplements.
Position Overview
The Community Development Worker promotes wellness through social, recreational, and educational activities for onsite tenants. Responsibilities include networking with external organizations, developing and evaluating programs, supervising students and volunteers, and assisting in policy development to enhance service delivery.
What You Will Do
- Maintain records related to membership and activities using client database software.
- Assist with preparing reports and statistics to ensure compliance with funder guidelines.
- Identify emerging needs and develop new programs in collaboration with the manager.
- Ensure organizational anti-racism objectives and community development principles are upheld.
- Familiarize with emergency procedures and service standards.
- Plan and facilitate onsite activities with staff and external partners.
- Engage tenants to assess interests and develop programming accordingly.
- Build healthy, engaging program spaces and facilitate workshops.
- Create and manage programming calendars and marketing materials.
- Implement participant surveys and gather feedback.
- Provide emotional support and encourage participation, collaborating with case managers.
- Work with community partners to maximize support and create pathways to recovery.
- Ensure principles of anti-oppression, community development, and harm reduction are integrated.
- Participate in training, supervision, and performance reviews.
- Document interactions and manage statistical data using software.
- Be prepared to work in environments with pest issues, hoarding, death, and drug use.
- Perform other duties as required.
What You Bring to the Team
- Bachelor’s degree in social work, community development, or related field.
- 3+ years creating and facilitating programs in social services.
- 2+ years designing health promotion and recreation programs.
- Experience with needs assessments and program planning.
