A community health organization in Toronto seeks a full-time Case Manager to provide comprehensive case management support to vulnerable residents. You will facilitate ongoing assessments, create service plans, and help clients navigate resources. Ideal candidates should have a degree in human services and at least two years of relevant experience. Flexible work hours and a supportive work environment are offered.

Qualifications
Minimum of two years experience with homeless or low-income individuals.
Direct experience with housing services in Scarborough is an asset.
Responsabilités
Provide advocacy and support to clients and families.
Facilitate assessments and service plans for clients.
Develop tenant support and goal plans.
Coordinate with community partners for referrals.
Connaissances
Knowledge of low income and vulnerable populations
Conflict resolution skills
Case management skills
Understanding of harm reduction
Excellent communication skills
Crisis intervention skills
Proficiency in various software applications
Formation
Degree or diploma in human services field
