Aboriginal Health Centre is a non-profit organization dedicated to enhancing the wellness of the Indigenous community through holistic, culturally appropriate healthcare programs and services. These include primary health care, traditional healing, mental health support, outreach, and health promotion. The Diabetes Education Program offers clinical care and education to patients with diabetes and those at risk. The Registered Dietitian will provide evidence-based, client-centered guidance on dietary management of diabetes in individual and group settings.

Responsibilities

1. Provide one-on-one counseling to patients living with diabetes.
2. Handle general referrals from the Primary Care Department.
3. Develop, implement, and evaluate nutrition care plans for diabetic patients.
4. Create, implement, and assess an in-house weight management program.
5. Facilitate group education sessions on healthy eating and diabetes management.
6. Maintain up-to-date medical records, monitor clients, and communicate with team members to ensure continuity of care.
7. Create educational resources as needed.
8. Perform administrative tasks such as data entry and report submissions, and contribute to quarterly program reports.
9. Participate in team and network-wide meetings.
10. Contribute to the development, implementation, monitoring, and evaluation of the Diabetes Management Program.

Program

• Maintain a culturally sensitive and evidence-based approach.

Qualifications

Education:

• Bachelor’s degree in Food and Nutrition and completion of an accredited Dietetic Internship.
• Registered with the College of Dietitians of Ontario.
• Knowledge and proficiency in diabetes care; CDE certification is an asset.

Experience:

• 1-5 years in diabetes care or related fields.
• Excellent communication and interpersonal skills.
• Experience working with Indigenous clients.
• Strong organizational and time management skills.
• Experience applying Diabetes Clinical Practice Guidelines.
• Experience working under a medical directive and with complex care portfolios.
• Sensitivity to barriers faced by clients and communities.
• Experience in health promotion and illness prevention initiatives.

Additional Skills:

• Administrative experience.
• Planning, development, and evaluation skills.
• Ability to work independently and in teams.
• Experience with weight management and motivational interviewing is an asset.

Knowledge, Abilities, and Personal Qualities:

• Thorough knowledge of community-based healthcare.
• Effective communication and supervisory skills.
• Ability to work in a multidisciplinary environment and with Indigenous communities.
• Understanding of Indigenous traditional ways.
• Valid Class G driver’s license and access to a reliable vehicle for travel between sites.
