Job Description:
Responsible for supporting Medical Social Workers (MSW) team in direct patient work, administrative work and general duties
Manage routine cases assigned by MSWs independently
Provide training for new social work and administrative assistants
Conduct financial assessment on eligibility of applicants for the various financial schemes
Conduct field visits and facilitate discharges by accompanying patients home or to social service organizations
Able to independently take on inpatient and outpatient casework, doing follow up work for discharged patients and networking with community services to facilitate seamless care
Job Requirements:
