A healthcare institution in Singapore is looking for a Medical Social Work Support professional to assist the Medical Social Workers (MSW) team with direct patient work, administrative duties, and case management. The ideal candidate will have a diploma and at least one year of experience in a social service agency. Responsibilities include conducting financial assessments and facilitating patient discharges. This role offers a chance to impact patient care positively while working in a supportive environment.
Qualifications
- Minimum 1 year of experience in a social service agency.
- Certificate in basic counselling skills or related skills is an advantage.
Responsabilités
- Support Medical Social Workers (MSW) team in patient and administrative work.
- Manage routine cases assigned by MSWs independently.
- Conduct field visits and facilitate patient discharges.

Formation
- Responsible for supporting Medical Social Workers (MSW) team in direct patient work, administrative work and general duties
- Manage routine cases assigned by MSWs independently
- Provide training for new social work and administrative assistants
- Conduct financial assessment on eligibility of applicants for the various financial schemes
- Conduct field visits and facilitate discharges by accompanying patients home or to social service organizations
- Able to independently take on inpatient and outpatient casework, doing follow up work for discharged patients and networking with community services to facilitate seamless care
- Diploma with minimum 1 year of experience in social service agency
- Certificate in basic counselling skills or related skills will be added advantage
