Job description
Pharmacist- Nursing Home Care Coordination

We are seeking a patient-focused Community Pharmacist to coordinate pharmaceutical care between our community pharmacy and partnered nursing homes. The role emphasises patient-centred care, management of minor (micro) ailments, and coordination of medicines and consumable supplies to support safe and continuous care for elderly residents.

Key Responsibilities
Patient-Centred Care Coordination
  • Coordinate pharmaceutical care for residents across community pharmacy and nursing home settings
  • Communicate with doctors, nurses, caregivers, and family members to ensure continuity of care
  • Support medication adherence and patient education
  • Monitor for polypharmacy, inappropriate prescribing, drug interactions, and adverse drug reactions
  • Recommend medication optimization and deprescribing in collaboration with doctors
  • Support chronic disease management and geriatric pharmacotherapy
Micro-Ailment & Primary Care Support
  • Assess and manage minor / micro ailments (e.g. cough & cold, skin conditions, constipation, minor pain) within professional scope
  • Provide appropriate treatment recommendations and referrals when necessary
  • Promote safe self‑care and rational medicine use
Medication & Consumable Management
  • Arrange and coordinate supply of medicines, medical consumables, and health products for nursing home residents
  • Monitor stock usage, expiry dates, and timely replenishment
  • Liaise with suppliers and healthcare partners to ensure uninterrupted supply
Clinical & Professional Responsibilities
  • Provide medication counselling and basic medication review support
  • Maintain accurate documentation and patient records
  • Conduct regular medication reviews for elderly and frail residents
Requirements
  • Registered Pharmacist with Pharmacy Board Malaysia (PBM)
  • Clinical pharmacy experience preferred
  • Interest or experience in elderly care, nursing home services, or primary care is an advantage
  • Patient-centered, proactive, and well‑organized
  • Able to work independently and collaboratively
