What you will do:

• Conduct assessments of adults to identify needs and develop personalised care plans
• Collaborate effectively with hospital staff, healthcare professionals, and family members to facilitate smooth transitions from hospital to home

Requirements of the Social Worker:

• Degree or equivalent in social work
• Current registration with Social Work England
