Trainee Advanced Clinical Practitioner

The closing date is 05 October 2025

The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic, treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. Patients referred to the service are triaged to be seen within 2 hours, or same day for urgent assessment. The Hospital at Home Frailty Virtual Ward supports patients who would otherwise be in hospital to receive acute care and treatment in their own home. This approach reduces the risk of deterioration that may require an admission.

You will be required to work with a high degree of autonomy.

You will manage clinical care in partnership with individuals, families and carers, including analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.

You will exercise autonomy and decision making in complex, uncertainty and varying levels of risk, holding accountability for decisions made.

You will work alongside Consultant Practitioners, ACP's and other trainee ACP clinically.

Main duties of the job

Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.

*Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.

*Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments).

*Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make positive changes, using Health Education England's framework to promote person-centred approaches in health and care.

*Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

*Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, life style advice and care.

Person Specification
Qualifications
  • Appropriate professional qualification (nursing or allied health professional) that allows registration with the NMC or HCPC.
  • Professional UK registration e.g. NMC/HCPC.
  • Meet the criteria for entry on to a Centre for Advancing Practice accredited Advanced Clinical Practice (ACP) master's (level 7) programme,
  • OR
  • Registered on and currently completing a Centre for Advancing Practice accredited master's (level 7) ACP programme, OR in the process of gaining recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.
  • Able to demonstrate a good command of the English Language both written and verbal.
  • Computer Literate in both data entry and communicating via IT, emails and report.
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification.
  • Teaching and Assessing qualification or equivalent/mentorship qualification.
Experience
  • Evidence of developing specialised clinical skills across the relevant speciality.
  • Evidence of implementing change to promote high quality practice.
  • Evidence of establishing values based professional and evidence-based practice across pathways, services, and organisations.
  • System working with individuals, families, carers, communities, and others for population health and reducing inequalities.
  • Contribution to service development, e.g. applying quality improvement methodology.
  • Demonstratable experience of working within related areas and working at an appropriate senior level.

  • Evidence of working with institutes of Higher Education including lecturing.
