The Six Goals Programme brings together expertise from across NHS Wales, including Health Boards, NHS Trusts, NHS Wales Performance and Improvement, the Welsh Value in Health Centre, and Adult Social Care, alongside third-sector partners. Our focus is on delivering national strategic and ministerial priorities for Urgent and Emergency Care, specifically improving Hospital discharges using a Home First approach, reducing POCD, increasing Trusted Assessor, embedding D2RA and the Optimal Hospital Flow Framework across Wales.

NHS Wales Performance and Improvement works in partnership for and on behalf of Welsh Government, in and with the NHS in Wales and is hosted by Public Health Wales. Our key purpose is to drive improvements in the quality and safety of care — resulting in better and more equitable outcomes, access and patient experience, reduced variation, and improvements in population health. We do this by providing strong leadership and strategic direction, enabling, supporting, and directing NHS Wales to transform clinical services in line with national priorities and standards.
