A London-based healthcare provider is hiring a Community Support Assistant to join their Community Diabetes Team in Liverpool. The role involves delivering education sessions to patients with Type-2 Diabetes, using glucose monitoring devices, and collaborating with clinical teams. Candidates are expected to possess skills in supporting patients and managing diabetes effectively. Training will be provided. This position offers a diverse and rewarding experience in the healthcare field.
An exciting opportunity has arisen for a Community Support Assistant to be part of our Community Diabetes Team who are at the forefront of driving up standards and quality in diabetes care in Community and Primary Care Services across the city of Liverpool. The role will include working in a variety of other clinical areas, local communities, Primary Care Networks and patients' homes. Working in partnership with the diabetes teams of Liverpool University NHS Hospital Trust and services delivered by Mersey Care the role is diverse, varied and offers the opportunity for innovation and change. Working with our Specialist Nurses and Dietitians the post holder will deliver Information Sessions to patients living with Type‑2 Diabetes, including the use of new technology and options for simple dietetic changes packages, supporting our patients to effectively manage their diabetes. Training will be given in all areas.
