A healthcare organization in the UK is looking for Cancer Follow-up Coordinators to ensure effective patient follow-up after cancer treatment. The role involves tracking patients, coordinating care, and providing support to patients throughout their recovery journey. The Cancer Services Team are looking for Cancer Follow-up Coordinators. The Cancer Follow-Up Coordinators will work alongside Cancer Support Workers and Clinical Nurse Specialists along with the wider Cancer team to ensure patients have a timely, efficient, effective and supported follow-up period following completion of their cancer treatment. Post-holders will be based in a specific cancer - type team and work closely with the clinical and non-clinical members of that team.
These roles will be based across all our hospital sites - Royal Sussex County Hospital, Worthing Hospital and St Richard's Hospital.
This job description is an outline of the role and responsibilities. From time to time due to the needs of the service, the post holder may be asked to flexibly undertake other duties that are consistent with the role and banding, including project work, internal job rotation and absence cover.
