This is an exciting opportunity for a highly motivated and experienced General Practice Nurse to join our multi-disciplinary practice team.

We are a close working and dynamic team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

Due to our practice joining a Primary Care Network, we are looking to expand our practice team by seeking to employ an experienced General Practice Nurse under the ARRS scheme. Our team comprises of 4 GP Partners, 4 Salaried GPs, 3 Practice Nurses, 2 HCAs and a supportive and dedicated Administrative Team.

We have a strong culture of learning and staff developmentand will actively support and encourage development in any areas of specialinterest.

Main duties of the job

About you :

  • Experienced General Practice Nurse
  • Looking to work full time and with a desire to work as part of a supportive and enthusiastic team
  • To include work on occasional Saturdays and evenings on a rota basis for Extended Access cover
  • Desirable to have own transport and ability to drive to be able to offer support and care to housebound patients

Job responsibilities

JOB RESPONSIBILITIES:

  • Adhere to NMC revalidation requirements and practice within the NMC Code of Conduct
  • Comply with national infection control standards and health and safety guidelines, working to the IPC Policy
  • Consultwith patients and their families to undertake assessments of patient need
  • Supportin the long-term condition management within the organisation, including beinginvolved with quality improvement and assurance initiatives
  • Develop,implement and embed an effective care management programme for the frail andhousebound patients registered with the organisation
  • Supportpublic health and screening programmes, including immunisations, vaccinations,and cervical screening and also provide opportunistic health promotion advice
  • Embedpopulation health management approaches within general practice, targetinghealth inequalities and improving access
  • Asrequired, provide routine nursing care to patients in accordance with clinicalbased evidence, NICE and the NSF
  • Supportnew-to-practice nurses and other healthcare professionals within theorganisation
  • Teachand advise patients and their families on how to manage their condition
  • Participatein clinical audits and research projects and implements changes as required,including supporting with the development, and updating of practiceprotocols/guidelines and procedures locally
  • Workwithin national and local protocols where these exist
  • Recognisethe boundaries of their practice and know when and to whom patients should bereferred
  • Supportin the delivery of enhanced services and other service requirements
  • Beaware of duties and responsibilities regarding current legislation and adhereto practice policies and procedures on Safeguarding Adults and SafeguardingChildren
  • Participatein the management of patient complaints when requested to do so and participatein the identification of any necessary learning brought about through incidentsand near-miss events
  • Undertakeall mandatory training and induction programmes
  • Contributeto and embrace the spectrum of clinical governance
  • Supportthe practice team with the reviewing and implementation of practice policiesand protocols, ensuring conformance to extant legislation
  • Attenda formal appraisal with their manager at least every 12 months. Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed
  • Provide immunisations including routine influenza, pneumococcal, shingles and RSV vaccinations
  • Provide travel advice and immunisation services
  • Perform cervical smears, pregnancy tests and give contraceptive advice
  • Assist GPs with minor surgeries and coil fittings if required
  • Carry out ECGs, venepuncture, urinalysis and wound care (including ulcer/Doppler management and suture removal)
  • Maintainaccurate clinical records in conjunction with extant legislation ensuring thatSNOMED CT codes are used effectively
  • Contributeto practice targets (QOF, etc.), complying with relevant guidance
  • Maintainchronic disease registers and implement an effective call/recall system
  • Perform practice and housekeeping duties, maintaining a clean, tidy, effective working area at all times
  • Monitor and order medical stock, drugs and vaccinations
  • Chaperonepatients where necessary
  • Supportthe team in dealing with clinical emergencies
  • Liaisewith external services/agencies to ensure the patient is supportedappropriately (vulnerable patients, etc.)
  • Attend and contribute to practice meetings and training sessions
  • Conduct home visits as required

Theremay be, on occasion, a requirement to carry out other tasks, this will bedependent upon factors such as workload and staffing levels

Person Specification
Experience
  • Previous primary care experience, specifically working within a GP practice nursing team
  • Experience of NHS IT System EMIS.
  • Good level of communication skills both verbal and written which can be tailored to a wide range of stakeholders.
  • Good level of ICT knowledge and skills.
  • Good organisational skills and time management.
  • Professional, calm and efficient manner.
  • Self-motivated, can work well independently and within a team.
  • Works safely and effectively when working autonomously and in conjunction with other health care professionals
  • Experience of clinical audit, evaluation research and implementation of evidence based practice
  • Understanding of national and local guidelines
  • Understand clinical governance and risk management
Qualifications
  • Hold one or more academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives.
  • Registered with the NMC and maintains revalidation in line with NMC requirements.
  • Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
  • Enhanced DBS check
  • Evidence of post qualifying and continuing professional development
  • Valid UK driving licence
