A healthcare provider in the UK is looking for an experienced Nurse to join their dynamic team. The role involves managing care for residents in care homes, conducting clinical assessments, and working collaboratively with a multi-disciplinary team. The post holder will be required totravel in between care homes and have a flexible approach to their day. They must hold a full drivers licence andaccess to own transport.
The post holder may also be expected to hold clinics for noncare home patients at the surgery and work collaboratively with Bethesda's nursing team, carrying out general practice nursing duties efficiently.
The successful candidate will be partof a team that will work with care homes to enhance quality of care and willhave experience of working within a wider healthcare team.
The candidate will also have experience inLong Term Condition Management.
Bethesda Medical Centre is a large surgery based on the cliff tops in Margate, Kent.
We have approxiamately 19,000 patients and have strong work ethics to provide good patient care through teamwork .You will be part of an established and professional nursing team, Acute Care Team, HCA's , GP's ,Frailty Team and Mental Health Nurses
JobDescription
Dutieswill include:
Managinga case load of care home residents across a number of care homes.
Providecare for the presenting patient, to include initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care.
Demonstratesafe, clinical decision making including assessment and diagnosis skills
Carryingout regular planned (and in some cases) acute emergency visits
Producingand reviewing ACPs, TEPs (RESPECT) and promote Restore 2 training.
Proactivelyplanning patient care and assessing ongoing patient needs
Regularliaison with assigned GPs whenever appropriate
Proactivelyworking with Care Home and Nursing Home staff and management to promote ongoingplanned patients care
Discussingend of life care with patients and relatives
Coordinatingreferrals to other services such as, chiropodist, dietician and tissue viability.
Workingwithin their professional boundaries
Beinvolved in weekly care home rounds
The post holder may also be expected to hold clinics for noncare home patients at the surgery and work collaboratively with Bethesda's nursing team, carrying out general practice nursing duties efficiently.
