A healthcare provider in Sheffield is seeking a qualified Practice Nurse to join their growing team. The role involves providing nursing and health promotion services, managing chronic diseases, and collaborating with a multidisciplinary team. Candidates should have significant experience in a primary care setting and be registered with the NMC. A proactive and motivated approach is essential for this role.
We are delighted to announce that we have a position to join our team as a Practice Nurse. We have a growing list of 30,000 patients, with a diverse mix of inner city residents and students from Sheffield Hallam University.
Our large expanding team consists of Doctors, Advanced Clinical Practitioners, Nursing team (NP, PNs, HCAs), Clinical pharmacy team (2 CPs and 2 pharmacy technicians) and first contact Physiotherapists.
We work very closely with Sheffield Hallam University, and have a medical centre housed in the heart of the university.

We are looking for a proactive, reliable and forward thinking practice nurse to join our team.
The successful candidate will form part of the nursing team in providingnursing and health promotion services to the Practice population. The candidate will provideholistic care to patients with or without direct referral from a doctor orsenior nurse, within clinical guidelines.
The Practice Nurse will be responsible for several clinical areas such as COPD and Diabetes as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Responsibilities
Thefollowing are the core responsibilities of the practice nurse (prescriber). Theremay be on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:
Clinical
Toassess, plan, implement and evaluate patient care
Tobe involved in immunisation, including campaigns as required by the publichealth needs of the Practice population
Toexercise knowledge and skills in chronic disease, wound assessment and management,ear care, contraception, and other clinical skills as appropriate to the role
Toparticipate in infection control
Toparticipate in the screening and health promotion activities of the Practice
Todevelop skills in sexual health, including cervical cytology and sexual healthscreening
Toparticipate in chronic disease management within Practice guidelines
Tomake referrals to, and receive referrals from other health care professionals
Tom demonstrate a level of knowledge clinical skill appropriate to the needs of therole
Toprescribe medications within competence and scope of practice
Management
Toparticipate in the development of Practice philosophy, strategy, and policy
Tobe involved in the management of change within the Practice and theimplementation of systems to improve patient care
Toparticipate in, and contribute to, the clinical governance agenda of thePractice
Toassist the Practice in achieving the Quality and Outcome Framework (GMScontract)
Toevaluate and audit care
Toparticipate in multidisciplinary protocol development
Tobe responsible for the maintenance of supplies and equipment
Training and education
Tocontribute to the training of GP Registrars, Medical and Nursing students, andother students and members of staff as required
Toparticipate in the clinical meetings within the Practice
Tomaintain own professional development and PDP
Toattend study days, courses and other educational events as required by the role
Toparticipate in the Practice appraisal process
Communication
Tomaintain accurate and contemporaneous records
Towork with the wider nursing team in developing an integrated service forpatients
Tomake referrals to other members of the Practice team, and to voluntary andstatutory agencies as necessary
Professional
Towork within the NMC Code of Professional Conduct and Scope of ProfessionalPractice
Toundertake professional development as required by the role
To undertake an annual non-medical prescribingaudit.
