Join us as a Band 5 Staff Nurse, full-time, in a substantive position. Our service operates from 8am - 6pm, seven days a week, offering flexibility in working hours. An additional team of Nurses and HCA's offer a robust, reactive evening and night Nursing team.
You will be responsible for providing skilled nursing care and will work flexibly supporting the team to deliver high quality integrated care. You will need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable.
Providing integrated care following a holistic assessment which enables patients to be cared for in their own homes and clinic settings. You will facilitate the prevention of unnecessary hospital admission/re-admission by reducing the effects of ill health and disability and maximising the individual's potential for independence.
A robust induction and competency-based development package will be provided. Newly qualified staff will be offered a preceptorship programme.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
