A vacancy at Birmingham Community Healthcare NHS Foundation Trust. You will join the Early Intervention Community Team within the Trust, part of a wider Early Intervention programme that has delivered substantial reductions in bed days and significant cost savings across Birmingham.

You will be joining a large, multi-partner transformation in health and social care, focused on preventing avoidable hospital admissions, facilitating early discharge, and delivering high quality care in patients\' homes and community settings.

• Lead and promote a culture of home first rehabilitation within a self-motivated and aspirational MDT.
• Work multi-professionally to complete assessments and deliver interventions, ensuring high quality care in patients\' homes and other community settings.
• Support junior staff and students, and drive service development projects and quality patient care.
• Communicate effectively with Health and Social care partners and uphold trusted assessor principles.

• HCPC or NMC registration is a requirement for registered staff roles.
• Excellent communication skills with a flexible approach and established or developing links with Health and Social care partners.
• Willingness to work shifts across 7 days per week in line with service delivery.
